How Can I generate PWM Signal on FPGA?

Issue Details

I am looking for a simple method to output a PWM Signal using the FPGA, I tried using few example codes however they don't work the way I want, I would like to be able to specify the frequency of the signal in Hz and the duty cycle in percentage, where can I find code like this?
